Choose a week below. Daily care is $50 per day. Daily care is provided on Monday, Tuesday, Thursday, and Friday. Daily care starts the second week of camp.

Will your child need after care? Type YES or NO. After care is 3pm - 6pm $10 per hour.

Dismissal is 3pm. Please list THREE individuals that are allowed to pick up your child. Any individual not listed below must be pre approved.

Daily Care runs from July 6th - July 31st. A deposit of $50 is needed to hold your child’s placement for the day. (Nonrefundable) After the completion of this form you will receive an email from AMB Staff with further details. Do you understand?
